Is Modafinil safe?
Modafinil
Modafinil (Provigil, Modiodal) is a wakefulness drug with a wide overdose margin and lower misuse potential than amphetamine-type stimulants. The main risks are rare but severe skin reactions, psychiatric effects, a possible risk of birth defects and an interaction that can make hormonal contraception fail.

How the score breaks down
The overall score is the weighted average of these factors. Scoring rules
Serious side effects30% of score
70 out of 100, GoodHeadache (34% vs 23%) and nausea (11% vs 3%) lead in US trials. Serious risks are rare but severe, including SJS, TEN and DRESS, psychiatric reactions such as mania, psychosis and suicidal thoughts, and a possible risk of birth defects (MHRA, 2020).
Overdose margin20% of score
82 out of 100, GoodThe label reports single overdoses of 4,000 and 4,500 mg (20 times the usual dose) without life-threatening effects, but also fatal overdoses after marketing involving modafinil alone or with other drugs. Agitation, insomnia, fast heart rate and chest pain are typical.

When another drug or supplement speeds them up or slows them down, drug levels change.More in the glossary, which can make the pill and other hormonal contraceptives fail during use and for a month after (two months in UK advice). It lowers cyclosporine levels, raises CYP2C19 drugs such as phenytoin, diazepam and omeprazole, and needs extra INR checks with warfarin.
Dependence and misuse15% of score
68 out of 100, FairControlled substanceA medicine or drug that the law restricts because it can be misused or cause dependence. Countries group them into schedules or classes.More in the glossary. In a study of experienced drug users, 200 to 800 mg produced euphoria and effects like methylphenidate. A 9-week trial found no withdrawal symptoms, though sleepiness returned in people with narcolepsy. Misuse risk is lower than for Schedule II stimulants.
Regulatory warnings10% of score
55 out of 100, FairNo Boxed warningThe strongest safety warning the US FDA puts on a medicine's label, printed inside a black border at the top.More in the glossary, but in 2010 the EMA restricted modafinil to narcolepsy, citing skin, psychiatric and heart risks and weak evidence for other uses. The MHRA warned in 2020 against use in pregnancy. Not approved for children in the US.

Is modafinil safe?
When prescribed for the conditions it is approved for, modafinil is one of the better-tolerated wakefulness drugs, and much harder to overdose on than amphetamine-type stimulants. Sold as Provigil and as generics, it treats excessive sleepiness in adults with narcolepsy, obstructive sleep apnoea or shift work disorder in the US. In Europe and the UK it has been approved for narcolepsy only since a 2010 review.
The catches are rare but serious. Modafinil can cause severe skin reactions, can trigger psychiatric symptoms, may raise the risk of birth defects and can make hormonal contraception fail. We score it 71, well above methylphenidate (58) and lisdexamfetamine (55), mainly because of its wider Overdose marginHow far a normal dose is from a harmful one. A wide margin means an accidental extra dose is unlikely to cause serious harm.More in the glossary and Controlled substanceA medicine or drug that the law restricts because it can be misused or cause dependence. Countries group them into schedules or classes.More in the glossary status.
What dose of modafinil is normal?
The US Provigil label gives one dose for all three uses: 200 mg once a day. For narcolepsy and sleep apnoea it is taken in the morning; for shift work disorder, about an hour before the shift starts. Doses up to 400 mg a day have been well tolerated, but the label says there is no consistent evidence they work better. People with severe liver disease take half the dose, and older adults may need less.
In sleep apnoea, modafinil treats the sleepiness, not the blocked airway. The label asks that CPAP be tried properly first and continued.
Rash, mood changes and the heart
Most side effects are mild. In pooled US trials, headache affected 34% of people on modafinil against 23% on placebo, and nausea 11% against 3%. Anxiety, insomnia and nervousness were also more common. Eight per cent stopped for side effects, against 3% on placebo.
The serious risk is the skin. In trials in children, 0.8% (13 of 1,585) developed a rash bad enough to stop treatment, including one possible case of Stevens-Johnson syndrome. After marketing, rare cases of SJS, toxic epidermal necrolysis and DRESS were reported in adults and children, at a reporting rate above the background rate. Most began within 1 to 5 weeks. The label says to stop at the first sign of rash unless it is clearly unrelated, and modafinil is not approved for children for any use.
Psychiatric reactions are the second concern. Reports after marketing include mania, delusions, hallucinations, suicidal thoughts and aggression, some needing hospital care. Many, though not all, of those people had a psychiatric history. The label also advises against modafinil in people with left ventricular hypertrophy or a past reaction to stimulants linked to mitral valve prolapse.
Contraception and pregnancy
This is the interaction most often missed. Modafinil induces CYP450 enzymesA family of liver enzymes that break down many medicines. When another drug or supplement speeds them up or slows them down, drug levels change.More in the glossary, the liver enzyme that clears oestrogen and progestogen, so the contraceptive pill and other hormonal methods may stop working. The US label advises an alternative or extra method during treatment and for a month after stopping. The MHRA's 2020 advice says two months.
That matters because of the second 2020 finding. A European review of a US pregnancy registry and individual reports found a possible higher risk of major birth defects, including heart defects, hypospadias and cleft lip or palate. The MHRA now says modafinil should not be used in pregnancy. The US label still uses the older Category C wording and mentions reports of restricted fetal growth and miscarriage.
Modafinil also lowers cyclosporine levels, can raise levels of phenytoin, diazepam, propranolol and omeprazole, and calls for more frequent INR checks with warfarin.
Smart drug use and online pharmacies
Modafinil is widely used without a prescription by students and shift workers hoping to focus or stay awake. A 2015 Oxford review of studies in healthy, rested adults found some gains on complex attention and planning tasks, with mixed results on simpler ones. No regulator has approved that use, and the evidence for it is thin.
The bigger risk is the supply. In 2016 the MHRA warned students against buying "smart drugs" online and reported seizing more than 31,000 doses of narcolepsy medication in one international operation. Modafinil has kept turning up in UK cases since: in an illegal online supply ring convicted in November 2025 and among 130,000 doses seized in a Bolton network whose members were sentenced in April 2026. Tablets from these sources may contain the wrong dose or something else entirely, and come with no screening for rash, heart or psychiatric risk.
The FDA's BeSafeRx campaign lists the signs of a legitimate US online pharmacy: it always asks for a prescription, is licensed by a state board of pharmacy, has a US address and phone number, and has a pharmacist you can talk to.
Who should be careful
Talk to your doctor or pharmacist before taking modafinil, or before adding another medicine, if you:
- use hormonal contraception, or could become pregnant
- have had a serious rash with any medicine, including armodafinil
- have had depression, mania or psychosis
- have heart disease, high blood pressure or a heart rhythm problem
- take cyclosporine, warfarin, phenytoin or a benzodiazepine
- have severe liver disease
In an overdose, call 911 or Poison Help on 1-800-222-1222 in the US, or 999 in the UK. Do not stop or change a prescribed dose on your own; plan any change with your prescriber.
How we scored it
Track record (88) and Overdose marginHow far a normal dose is from a harmful one. A wide margin means an accidental extra dose is unlikely to cause serious harm.More in the glossary (82) are the strongest factors. Regulatory warnings (55) are the weakest, reflecting the EU restriction to narcolepsy and the UK pregnancy warning, followed by interactions (62), driven by the contraceptive effect.
Questions people ask
Does modafinil make birth control less effective?
Yes, it can. Modafinil speeds up the breakdown of the hormones in the pill and other hormonal contraceptives. The US label advises another or additional method during treatment and for a month after stopping; UK advice says two months. Talk to your doctor or pharmacist about which methods still work.
Is it legal to buy modafinil online?
In the US it is a Controlled substanceA medicine or drug that the law restricts because it can be misused or cause dependence. Countries group them into schedules or classes.More in the glossary drug and needs a prescription; a pharmacy that sells it without one is breaking the law. In the UK it is Prescription-only medicineA medicine that can be supplied only with a prescription from a doctor or another authorised prescriber.More in the glossary, and the MHRA says websites supplying it without a valid prescription are acting illegally. Tablets from such sites may not contain what the label says.
Does modafinil work as a smart drug?
A 2015 Oxford review of studies in healthy, rested adults found improvements in some tests of attention, planning and learning, with mixed results on simpler tasks. The effects were modest and the studies small and short. It is not approved for this use anywhere.
What is the difference between modafinil and armodafinil?
Armodafinil (Nuvigil) contains only the R-form of modafinil, which stays in the body longer. It is approved in the US for the same three conditions at 150 to 250 mg a day and carries the same warnings on rash, psychiatric effects and contraception.
How long does modafinil stay in your system?
The label gives an effective half-life of about 15 hours after repeated doses, which is why it is taken in the morning or before a night shift. Taken later in the day it can disturb sleep.
