Is Rivaroxaban safe?

Rivaroxaban

Rivaroxaban (Xarelto) is a once-a-day blood thinner with no INR tests. In its main trial it caused fewer brain and fatal bleeds than warfarin but more stomach and gut bleeding. The 15 mg and 20 mg tablets must be taken with food, and the US label carries a boxed warning on stopping early and spinal procedures.

How the score breaks down

The overall score is the weighted average of these factors. Scoring rules

  • Serious side effects30% of score

    48 out of 100, Weak

In ROCKET AF, major bleeding was 3.6% a year against 3.5% on warfarin, gut bleeding was higher (2.0% vs 1.2%), while bleeding into the brain (0.5% vs 0.7%) and fatal bleeding (0.2% vs 0.5%) were lower (US label, NEJM 2011).

  • Overdose margin20% of score

    58 out of 100, Fair

Fixed doses need no monitoring, and absorption levels off above single doses of 50 mg. But it cannot be removed by dialysis, and the September 2026 US label states no specific reversal agent is available.

  • Interactions15% of score

    52 out of 100, Weak

Is rivaroxaban safe?

Rivaroxaban, sold as Xarelto, is an effective blood thinner that trades one bleeding risk for another. In ROCKET AF, the 14,264-patient trial against warfarin in atrial fibrillation, overall major bleeding was about the same (3.6% against 3.5% a year). Bleeding into the brain and fatal bleeding were lower on rivaroxaban, but stomach and gut bleeding was higher.

We score it 61, below apixaban (64) because apixaban cut major bleeding in its own trial and rivaroxaban did not, and well above warfarin (49) because it needs no INR tests and caused fewer of the worst bleeds. A middling score does not mean it should be stopped. Stopping without another anticoagulant raises the risk of stroke, which is one of the two parts of its Boxed warningThe strongest safety warning the US FDA puts on a medicine's label, printed inside a black border at the top.More in the glossary.

How does rivaroxaban compare with warfarin?

The US label gives the bleeding results from ROCKET AF:

Per yearRivaroxabanWarfarin
Major bleeding3.6%3.5%
Bleeding into the brain0.5%0.7%
Stomach and gut bleeding2.0%1.2%
Fatal bleeding0.2%0.5%

For preventing strokes, rivaroxaban was at least as good as warfarin: 1.7% against 2.2% a year in the main per-protocol analysis. More people stopped rivaroxaban because of bleeding (4.3% against 3.1%). For someone with a history of stomach ulcers or gut bleeding, that row of the table matters most, and it is worth raising with the prescriber.

The food rule

The 15 mg and 20 mg tablets need food to be absorbed properly. In 2019 the UK's MHRA issued a reminder after a small number of reports of clots in people who had taken these doses on an empty stomach. The US label says to take the atrial fibrillation dose with the evening meal. The 2.5 mg and 10 mg tablets can be taken with or without food. If swallowing is hard, the tablets can be crushed and mixed with water or apple puree, followed by food.

Doses on the US label

  • Atrial fibrillation: 20 mg once a day with the evening meal, or 15 mg if creatinine clearance is 50 mL/min or lower.
  • DVT or pulmonary embolism: 15 mg twice a day with food for 21 days, then 20 mg once a day with food; 10 mg once a day to prevent a recurrence after at least 6 months.
  • After hip or knee replacement: 10 mg once a day for 35 or 12 days.
  • Coronary or leg artery disease: 2.5 mg twice a day with aspirin 75 to 100 mg.

Once-daily dosing is convenient, but the effect fades within about a day, so a missed dose leaves a gap. For once-daily doses the label says to take a missed tablet as soon as you remember, never two on the same day.

Interactions and other blood thinners

Rivaroxaban has no food interactions beyond the absorption rule, and far fewer drug interactions than warfarin. The label says to avoid ketoconazole and ritonavir, which raise levels, and rifampin, carbamazepine, phenytoin and St John's wort, which lower them. Erythromycin is a problem in people with reduced kidney function.

The bigger everyday risk is stacking bleeding drugs. Aspirin, clopidogrel, NSAIDs such as ibuprofen and naproxen, and SSRI or SNRI antidepressants all add to it. Some people with heart disease take rivaroxaban with aspirin on purpose, at the low 2.5 mg dose; that is a decision for the cardiologist.

Overdose and reversal

Very large doses are partly self-limiting, because the gut absorbs little more above about 50 mg. Rivaroxaban still cannot be removed by dialysis, and the September 2026 US label says no specific reversal agent is available, after the antidote andexanet alfa left the US market in December 2025. Hospitals use clotting-factor concentrates. For a suspected overdose or a child who swallowed tablets, call Poison Control (1-800-222-1222) in the US or NHS 111 in the UK.

Who should be careful

  • People with prosthetic heart valves, including after TAVR: the label does not recommend rivaroxaban, because the GALILEO trial found more deaths and bleeding.
  • People with antiphospholipid syndrome, severe kidney disease or a history of gut bleeding.
  • Anyone having surgery, an epidural or a spinal injection: the label advises stopping at least 24 hours before procedures, as decided by the team.
  • Pregnant women: the label warns of bleeding around delivery.

Talk to your doctor or pharmacist before stopping, switching or adding any medicine.

How we scored it

Dependence (98) and track record (87) are strong. Serious side effects (48) reflect bleeding rates similar to warfarin with more gut bleeding, and regulatory warnings (38) reflect the Boxed warningThe strongest safety warning the US FDA puts on a medicine's label, printed inside a black border at the top.More in the glossary and valve restriction. Interactions (52) and Overdose marginHow far a normal dose is from a harmful one. A wide margin means an accidental extra dose is unlikely to cause serious harm.More in the glossary (58) are moderate, held back by the food rule and the lack of a US antidote.

Questions people ask

Do I have to take Xarelto with food?

The 15 mg and 20 mg tablets, yes. They are absorbed much better with a meal, and the UK regulator warned in 2019 about clots in people who took them on an empty stomach. The US label says to take the atrial fibrillation dose with the evening meal. The 2.5 mg and 10 mg tablets can be taken with or without food.

Is Xarelto or Eliquis safer?

They have never been compared head to head in a large randomised trial. Against warfarin, apixaban reduced major bleeding while rivaroxaban did not, and rivaroxaban caused more gut bleeding. Rivaroxaban is once a day, which some people find easier. Which suits you depends on kidney function, other medicines and bleeding history, so ask your doctor.

What if I forget a dose of rivaroxaban?

For once-daily doses, the US label says to take the missed dose as soon as you remember, but not to take two doses on the same day. Rules differ for the twice-daily doses, so check your leaflet or ask a pharmacist.

Can I take ibuprofen with rivaroxaban?

NSAIDs such as ibuprofen and naproxen raise the risk of stomach bleeding when combined with any blood thinner, and the label lists them as a bleeding risk. Paracetamol (acetaminophen) is usually the preferred painkiller. Check with a pharmacist before taking anything new.

Is there an antidote for Xarelto?

The current US label says no specific reversal agent is available. The antidote andexanet alfa was withdrawn in the US in December 2025, though it is still sold in the UK and EU. Rivaroxaban wears off within a day or two, and hospitals use clotting-factor concentrates for life-threatening bleeding.
