Is Caffeine pills safe?
Caffeine pills
Caffeine tablets taken as labelled, 100 to 200 mg at a time, are about as safe as coffee for healthy adults and the dose is more exact. The catch is the narrow gap to harm. A handful of tablets can cause seizures and heart rhythm problems, and fatal overdoses are mostly from tablets and powders.

How the score breaks down
The overall score is the weighted average of these factors. Scoring rules
Safety at usual doses25% of score
68 out of 100, FairAt 100 to 200 mg, within EFSA's 200 mg single-dose and FDA's 400 mg daily guidance. The margin is narrow. The FDA says about 1,200 mg taken quickly can cause seizures, which is six 200 mg tablets.
Organ toxicity reports20% of score
62 out of 100, FairNo liver or kidney injury at normal doses, but overdose causes seizures and fatal arrhythmias. A 2018 review counted 92 deaths with caffeine as the only cause; a Maryland case series found tablets were the most potent source.

When another drug or supplement speeds them up or slows them down, drug levels change.More in the glossary raise caffeine levels, per the ciprofloxacin label. Effects add up with stimulant medicines, decongestants and caffeine in drinks and painkillers.

Caffeine is cheap and easy to dose exactly; the quality risk lies in bulk powders and supplement versions.

Pure powders are restricted. The FDA said in 2018 that bulk pure caffeine supplements can be adulterated, and Australia and New Zealand banned their retail sale from late 2019.
Are caffeine pills safe?
Taken as labelled, yes, for most healthy adults. A caffeine tablet holds 100 to 200 mg, about the same as one or two cups of brewed coffee, and the US Over-the-counterA medicine you can buy without a prescription, in a pharmacy or shop, with directions on the pack for safe use without a doctor.More in the glossary rule limits use to that amount no more often than every three to four hours. That keeps a user inside the FDA's figure of about 400 mg a day and EFSA's 200 mg per dose.
The problem is the margin. The FDA says about 1,200 mg of caffeine taken quickly can cause seizures, and that is six 200 mg tablets. Few people overdose on coffee because the volume stops them. Nothing stops you swallowing a handful of tablets, which is why caffeine pills score 69 here, below melatonin and well below everyday vitamins.
How much caffeine is too much?
| Amount | What it means |
|---|---|
| 100 mg | EFSA: close to bedtime, enough to disturb sleep |
| 200 mg (about 3 mg/kg) | EFSA: safe single dose for healthy adults, including before exercise |
| 200 mg a day | EFSA: limit for pregnant and breastfeeding women |
| 400 mg a day | FDA and EFSA: not usually linked to harm in healthy adults |
| About 1,200 mg taken quickly | FDA: can cause seizures |
Side effects start well before the danger zone: jitteriness, anxiety, a fast heartbeat, stomach upset and poor sleep. People vary a lot. Slow metabolisers, people with anxiety disorders and those who rarely drink coffee feel a 200 mg tablet much more than a regular coffee drinker.
Count caffeine from every source. Coffee, energy drinks, pre-workout powders and some painkillers and cold remedies add up with tablets.
How do caffeine overdoses happen?
Fatal caffeine poisoning is rare, but it is almost always from concentrated forms. A 2018 systematic review in Nutrients found 92 published deaths where caffeine was the only cause, with infants, people with psychiatric illness and athletes over-represented. A Maryland medical examiner's series of eight deaths from 1999 to 2009 found that Over-the-counterA medicine you can buy without a prescription, in a pharmacy or shop, with directions on the pack for safe use without a doctor.More in the glossary tablets were the most potent source and that none of the deaths came from caffeine in food or drink. Many fatal cases are intentional overdoses, and a 2026 case report described a 25-year-old who died after taking about 60 tablets of 200 mg bought online.
Pure caffeine powder is the extreme case. Less than half a teaspoon can hold the 1,200 mg seizure dose, and kitchen measures cannot dose it safely. After deaths of young people in the US, the FDA said in April 2018 that it considers some bulk pure or highly concentrated caffeine supplements adulterated. Australia and New Zealand banned retail sale of such foods after a 21-year-old man in New South Wales died from caffeine toxicity linked to a powder.
Who should not take caffeine pills
- Children under 12. The US label says not to give them. EFSA's limit for children and teenagers is 3 mg per kilogram of body weight a day.
- Pregnant or breastfeeding women, for whom one tablet can reach the daily limit.
- People with heart rhythm problems, high blood pressure, anxiety disorders or seizure disorders. Talk to your doctor first.
- People taking ciprofloxacin or other drugs that slow caffeine breakdown, and those on stimulant medicines for ADHD such as amphetamine or methylphenidate. A pharmacist can check your list.
- Anyone with an eating disorder or depression, given how many fatal cases involve intentional overdose. Keep only a small supply at home.
How to use caffeine pills more safely
- Use 100 mg tablets if you can, or split a 200 mg one, and see how you react.
- Take one dose at a time and wait at least three to four hours before another.
- Stop by early afternoon if you want to sleep that night.
- Never buy pure caffeine powder or large tubs of tablets online.
- Get emergency help for vomiting, chest pain, a racing or irregular pulse, confusion or seizures after taking several tablets.
How we scored it
Product quality (82) is the strongest factor, because tablets are mostly regulated as Over-the-counterA medicine you can buy without a prescription, in a pharmacy or shop, with directions on the pack for safe use without a doctor.More in the glossary drugs with a fixed dose. Organ toxicity (62) and regulatory actions (65) are the weakest: overdose deaths come mainly from tablets and powders, and pure powders are restricted in the US, Australia and New Zealand. Dose safety (68) reflects a safe labelled dose with a narrow gap to harm. Compare green tea extract, where caffeine is one of several active ingredients.
Questions people ask
How many caffeine pills is too many?
For a healthy adult, the FDA puts the daily amount not usually linked to harm at about 400 mg, which is two 200 mg tablets in a day. EFSA advises no more than 200 mg at once. The FDA says seizures can occur at about 1,200 mg taken quickly, so never take several tablets together.
Are caffeine pills worse than coffee?
Milligram for milligram the caffeine is the same, and pills contain a known dose, which can be an advantage. The difference is that it is easy to swallow 1,000 mg of caffeine in tablets in a minute, which is hard to do with coffee.
Can you take caffeine pills every day?
The US label says they are for occasional use and not a substitute for sleep. Daily use within 400 mg is unlikely to harm a healthy adult but builds tolerance, and stopping causes headaches for a few days. If you need them to get through most days, talk to a doctor about why you are so tired.
Can caffeine pills cause heart problems?
At normal doses they can cause palpitations and a faster heart rate in sensitive people. In overdose, caffeine causes dangerous heart rhythms, which is how most caffeine deaths happen. People with heart rhythm problems or high blood pressure should ask their doctor first.
Are caffeine pills safe during pregnancy?
EFSA advises pregnant and breastfeeding women to keep caffeine from all sources to 200 mg a day, which a single tablet can reach. Talk to your doctor or midwife before using them.
What should I do if someone took too many caffeine pills?
Treat it as an emergency, even if they seem fine at first. Call emergency services or a poison centre (1-800-222-1222 in the US). Symptoms include vomiting, a racing or irregular heartbeat, agitation and seizures.
